It is understood that BSSNY Sports and Entertainment Inc and its affiliates (BSSNY) does not provide medical insurance covering injuries of any nature during the time of our camps/clinics. The undersigned certifies that the participant is cleared to participate in all camp related activities and hereby release BSSNY, its officials, employees or agents from any claims resulting from participation in BSS camps. I hereby authorized the Director of BSSNY to act with his best judgment.

It is also understood that from time to time BSSNY take pictures and videos of training sessions to help promote the program to potential clients/customers. By signing this document you are giving written permission to BSSNY to use the aforementioned pictures and videos
